Please read these brief policies to help us ensure a safe, professional, and therapeutic session for every client.
General Information
LifeSource Massage & Wellness LLC, referred to here as the practice, delivers expert-level massage therapy and spa services through practitioners fully licensed within the state of Florida. We ensure that every session adheres strictly to the legal frameworks established in Chapter 480 of the Florida Statutes. These professional guidelines serve as the foundation for our practice, emphasizing the preservation of public health and safety through rigorous adherence to state-mandated massage therapy regulations.
Eligibility & Health Disclosure
By booking an appointment, you acknowledge that you are at least 18 years of age or are accompanied by a parent or legal guardian. Understanding the importance of your wellness, you agree to provide complete and accurate health information. It is required that clients disclose all medical conditions, recent injuries, surgical history, pregnancy, and current medications. Please be aware that overlooking relevant health details may compromise your safety and influence the overall effectiveness of your treatment.
Informed Consent regarding your treatment is essential as massage therapy is a therapeutic wellness service rather than a substitute for professional medical care. By choosing to receive these services, you acknowledge that you have been fully informed about the nature of the therapy and are providing voluntary consent to the treatment. Please be aware that you maintain the right to withdraw this consent at any given time. Our therapists are committed to operating strictly within their licensed scope of practice while upholding all professional standards required under Florida law.
Massage therapy focuses on the manipulation of soft tissue for relaxation and therapeutic benefit; however, we do not diagnose medical conditions or prescribe medications. Our practitioners do not provide sexual or inappropriate services, and Florida law strictly prohibits any form of sexual misconduct in the practice of massage. We maintain the highest therapeutic standards by ensuring proper draping is used at all times and focusing treatment only on areas previously agreed upon. Any form of inappropriate behavior will be met with the immediate termination of the session. We further reserve the right to refuse or terminate our services due to misconduct, health and safety concerns, or any non-compliance with our established policies.
Appointments and Cancellation Policy
Please provide at least 24-hour notice if you need to cancel or reschedule your session. Late cancellations or missed appointments may incur a charge of up to 100% of the service cost. Should you arrive late, your session may be shortened to stay on schedule without a reduction in the full service fee.
Payment Policy. All payments are required in full at the time of your appointment unless the session has been prepaid. We accept several forms of payment for your convenience, including cash, credit cards, Google Pay, and Apple Pay. Please be advised that any service packages or promotional offers are non-transferable unless explicitly stated otherwise and remain subject to their designated expiration dates.
All services rendered by LifeSource Massage & Wellness are considered final and non-refundable. Regarding prepaid packages, any potential refund remains entirely at the provider’s discretion and will be calculated by deducting the full value of any services already utilized.
Health, Safety, and Privacy
Our facility adheres to strict professional sanitary practices that comply with all state regulations. We guarantee fresh, clean linens for every individual client and ensure that all equipment and treatment areas are thoroughly sanitized. Furthermore, massage establishments in Florida are subject to regular inspections and must consistently meet high sanitation and operational standards.
While massage therapy is generally considered safe, it does involve inherent risks that participants should be aware of. By choosing to receive services, you accept full responsibility for your personal health decisions and agree to release LifeSource Massage & Wellness LLC from all liability, except in specific cases involving practitioner negligence.
Protecting your privacy and maintaining absolute confidentiality is a priority. We only collect the specific personal and health information required to provide optimal care, and we do not disclose your data to third parties without your explicit consent unless we are legally mandated to do so.
Gift Certificates and Promotions
All promotional offers and gift certificates are non-refundable and must be utilized prior to their specified expiration dates. These credits hold no cash value and cannot be redeemed for currency unless otherwise mandated by applicable law.
