1. General Information LifeSource Massage & Wellness LLC (“we,” “our,” “us”) provides professional massage therapy services by a licensed massage therapist in the State of Florida. All services are performed in accordance with Florida law governing massage therapy (Chapter 480, Florida Statutes), which regulates massage practice to protect public health and safety

2. Eligibility & Health Disclosure By booking an appointment, you confirm that: You are at least 18 years old (or accompanied by a parent/guardian) You will provide accurate health information Clients must disclose: Medical conditions Injuries or surgeries Pregnancy Medications or other health concerns 👉 Failure to disclose relevant information may affect your safety and the effectiveness of treatment.

3. Informed Consent Massage therapy is a therapeutic wellness service and not a substitute for medical care. By receiving services, you acknowledge: You have been informed about the nature of massage therapy You voluntarily consent to treatment You may withdraw consent at any time Massage therapists are required to operate within their licensed scope of practice and maintain professional standards under Florida law

4. Scope of Practice (Important)Massage therapy involves the manipulation of soft tissue for relaxation and therapeutic benefit.We do NOT:Diagnose medical conditionsPrescribe treatment or medicationProvide sexual or inappropriate services⚠️ Florida law strictly prohibits sexual misconduct in massage therapy practice

5. Professional Boundaries & Conduct

  • Proper draping will be used at all times

  • Only areas agreed upon will be treated

  • Any inappropriate behavior will result in immediate termination of the session

We reserve the right to refuse or terminate service for:

  • Inappropriate conduct

  • Health safety concerns

  • Non-compliance with policies

6. Appointments & Cancellation Policy

  • 24-hour notice is required for cancellations or rescheduling

  • Late cancellations or no-shows may be charged up to 100% of service cost

  • Late arrivals may result in a shortened session without price adjustment

7. Payment Policy

  • Payment is due at the time of service unless prepaid

  • We accept cash, credit card, google pay, and apple pay

  • Packages and promotions:

    • Non-transferable unless specified

    • Subject to expiration dates

8. Refund Policy

  • Services rendered are non-refundable

  • Prepaid packages:

    • May be refundable at the provider’s discretion (less any services used)

9. Health & Safety

We maintain professional sanitary practices in compliance with state regulations:

  • Clean linens for each client

  • Sanitized equipment and treatment areas

Massage establishments in Florida are subject to inspection and must meet sanitation and operational standards

10. Privacy & Confidentiality

Your personal and health information is kept confidential.

We will:

  • Only collect information necessary for your care

  • Not share your information without consent, except as required by law

11. Gift Certificates / Promotions

  • Non-refundable

  • Must be used before expiration date

  • Cannot be redeemed for cash unless required by law

12. Liability Disclaimer

Massage therapy is generally safe but may involve inherent risks.

By receiving services:

  • You accept responsibility for your health decisions

  • You release LifeSource Massage & Wellness LLC from liability except in cases of negligence
